Can dependants apply at the same time as the main applicant?

Yes, and this is usually the simplest approach. Dependants can also apply later, as long as they apply before their current permission expires.

Are Home Office fees and the Immigration Health Surcharge included?

No. These are paid separately to the Home Office per applicant.
